Dengue fever is a viral infection transmitted by Aedes mosquitoes, primarily Aedes aegypti. The infection is caused by the dengue virus, of which there are four distinct serotypes: dengue-1, dengue-2, dengue-3, and dengue-4. Infection with one serotype provides lifelong immunity to that specific serotype, but a person can be infected again with a different serotype. A second dengue infection with a different serotype carries a significantly higher risk of developing severe dengue , dengue hemorrhagic fever or dengue shock syndrome.
Unlike malaria or typhoid fever, dengue has no animal or environmental reservoir outside humans. Transmission occurs exclusively through the bite of an infected Aedes mosquito. IgM and IgG Antibodies

IgG positivity alone indicates prior infection. The presence of IgG does not protect against infection with a different dengue serotype; a second infection with a different serotype carries a higher risk of severe dengue. Complete Blood Count (FBC)

Treatment
Supportive Care
There is no specific antiviral medication that kills the dengue virus. Treatment focuses on supporting the patient’s body while the immune system clears the infection. This includes maintaining adequate hydration, monitoring for warning signs of severe dengue, and managing fever and pain with paracetamol (acetaminophen). Aspirin and non-steroidal anti-inflammatory drugs like ibuprofen are avoided because they increase bleeding risk in dengue, particularly as the illness progresses.
Fluid Management
Maintaining hydration is critical. Patients are encouraged to drink oral rehydration solution or water frequently. In severe dengue with vomiting, inability to drink, or signs of shock, intravenous fluid replacement may be necessary. The goal is to maintain adequate circulation and organ perfusion without over-hydrating, which can worsen fluid accumulation in the lungs or abdomen.
Monitoring for Severe Dengue
The critical phase of dengue typically occurs as fever is resolving, often around day three to five of illness. This is when patients are at highest risk for dengue hemorrhagic fever or dengue shock syndrome. Close monitoring of vital signs, platelet counts, and clinical warning signs is essential. Patients presenting with warning signs or showing evidence of severe dengue require hospitalization for intensive monitoring and management.
Treatment Note
Unlike malaria or typhoid fever, there is no antibiotic or antiviral that treats dengue. Prevention
Reduce Mosquito Breeding Sites
Aedes mosquitoes breed in small collections of standing water around homes and communities. Eliminating these breeding sites is the most effective prevention strategy. Empty water from flower pots, discarded containers, roof gutters, and any other receptacles where water collects. Cover water storage containers tightly. Keep environments clean and dry.
Use Insect Repellents

Complications
Dengue Hemorrhagic Fever (DHF)
Dengue hemorrhagic fever represents a severe form of dengue infection characterized by abnormal bleeding, plasma leakage, and thrombocytopenia (low platelet count). DHF typically develops as the patient’s fever is resolving, creating a deceptive clinical situation where patients may appear to be improving while actually entering the most dangerous phase of illness. Warning signs include persistent vomiting, severe abdominal pain, bleeding, lethargy, and a rapid drop in platelet count. DHF requires hospitalization and intensive monitoring.
Dengue Shock Syndrome (DSS)
Dengue shock syndrome is the most severe form of dengue, characterized by circulatory collapse, profound hypotension, and organ dysfunction. DSS represents dengue hemorrhagic fever with additional signs of shock including weak or undetectable pulse, narrow pulse pressure, low blood pressure, cold extremities, and altered mental status. DSS is a medical emergency requiring immediate intensive care management, including aggressive fluid resuscitation and potentially vasopressors.

Differential Diagnosis
Malaria
Both malaria and dengue present with fever, headache, and body aches, making them clinically similar in the first days of illness. Key distinguishing features: dengue typically presents with an abrupt high fever and severe muscle and joint pain; malaria presents with fever and chills but less severe joint pain. Dengue rash appears around day three to four; malaria does not typically produce a rash. Dengue is transmitted by daytime-biting Aedes mosquitoes; malaria by night-biting Anopheles mosquitoes. Laboratory testing is essential to distinguish them , a negative malaria test does not rule out dengue, and the two can coexist in the same patient.
Typhoid Fever
Typhoid fever also presents with prolonged fever and general malaise but is distinguished by its gradual onset with a step-ladder fever pattern (unlike dengue’s abrupt presentation) and by constipation in early stages. Typhoid does not typically produce the severe joint pain characteristic of dengue. Laboratory testing , blood culture for typhoid, dengue testing for dengue , can distinguish between them.
Chikungunya
Chikungunya is another mosquito-borne viral illness transmitted by Aedes mosquitoes and presenting with fever, severe joint pain, and sometimes rash. The clinical presentation can be very similar to dengue. The joint pain in chikungunya often lasts much longer (weeks to months) compared to dengue. Laboratory testing can distinguish between them.
Other Viral Infections
Influenza, COVID-19, and other viral illnesses can present with fever and body aches but typically lack the severe joint pain and characteristic rash of dengue.

The similarity between dengue and malaria is particularly important in Ghana, where both diseases are now recognized. Dengue is increasing in urban areas while malaria remains endemic. Laboratory confirmation is essential to distinguish between them and to guide appropriate prevention strategies.
