Blood Culture

Laboratory Note
Antibiotics given before testing can make it harder for cultures to grow the bacteria in a lab. This is one reason treatment is not delayed while waiting for test results.
Treatment
Emergency Antibiotics
Started as soon as bacterial meningitis is suspected, before the exact bacteria is even confirmed, since delays of even a few hours are linked to worse outcomes.
Corticosteroids
Often given alongside the first dose of antibiotics to reduce inflammation and lower the risk of complications such as hearing loss.
Treatment Note
Bacterial meningitis is treated as a medical emergency. Care in a hospital, often with close monitoring, is needed for the full course of treatment.
Prevention
Get recommended vaccines
Vaccines against meningococcus, pneumococcus, and Haemophilus influenzae type b protect against the bacteria responsible for most cases.
Take preventive antibiotics after close contact
People who live with or have had close contact with someone diagnosed with bacterial meningitis may be offered antibiotics to prevent infection.
Practise good hygiene
Washing hands regularly and avoiding sharing cups, utensils, or toothbrushes reduces the spread of the bacteria that cause meningitis.
Seek screening during pregnancy
Testing for Group B Streptococcus and, when needed, antibiotics during labour can prevent it from passing from mother to newborn.
Prevention Note
In parts of sub-Saharan Africa known as the meningitis belt, large-scale vaccination campaigns have dramatically reduced epidemic meningitis and continue to expand.

Higher Risk of Severe Illness

Hearing Loss
One of the most common lasting effects of bacterial meningitis, sometimes permanent.
Neurological Effects
Seizures, limb weakness, and difficulties with vision, speech, or memory can persist after recovery.
Sepsis and Tissue Damage
In severe cases, infection spreading through the bloodstream can damage tissue, sometimes requiring amputation.

Differential Diagnosis
Malaria
Malaria can also cause fever and altered mental status in the same regions, and should be considered and ruled out, particularly where it is the dominant presumptive diagnosis.
Sepsis
Sepsis shares several overlapping features with bacterial meningitis and can also arise from it.
Typhoid Fever
Typhoid fever can cause fever and confusion in its later stages, though it typically develops more gradually than bacterial meningitis.
Clinical Note
Because bacterial meningitis can worsen within hours, urgent laboratory testing, particularly a lumbar puncture, is prioritised over waiting to distinguish it from these other causes of fever and confusion.
